The shape of the decision
Almost every South African safari holiday resolves into one of three choices: the Greater Kruger when depth of game viewing is the point, a malaria-free reserve when young children or medical caution are in play, or a Cape Town pairing when the trip has to work for travellers who are not all here for the wildlife.
The useful thing about South Africa is the range within each of those. A well-run Greater Kruger lodge and a Sabi Sand icon camp sit on the same ecosystem, so what you trade down for is finish, exclusivity and how many vehicles share a sighting, not whether you see anything.
Logistics are the other advantage. Everything connects on scheduled flights; small-aircraft charters exist here as an option rather than a requirement.
Where to actually go
The reserve decides your safari far more than the lodge does. This is how the main options differ.
The best leopard viewing on the continent, off-road traversing rights, and the blue-chip lodges to match. Priced accordingly, and worth it when game viewing is the reason for the trip.
The same unfenced ecosystem at gentler rates. The sensible choice when the budget has to stretch across more nights, or when the party wants a fifth night rather than a smarter suite.
Malaria-free, strong on wild dog and elephant, and about four hours by road from Johannesburg or a short charter. The usual answer for families who want Big 5 without antimalarials.
Malaria-free Big 5 reserves within reach of the Garden Route, which makes them the natural safari ending to a Cape trip rather than a separate journey.
Enormous, excellent value and genuinely wild, but you are on public roads, with the vehicle numbers that implies at a good sighting. We use it deliberately rather than by default.
Malaria-free and close to Johannesburg, which suits short add-on safaris and families with small children who cannot face a long transfer on arrival.

Health and honesty
Madikwe, Pilanesberg, the Waterberg and the Eastern Cape reserves carry no malaria risk, which settles the destination question for a good number of families. The Greater Kruger regions are classified low-risk and seasonal rather than malaria-free, so the answer there depends on your month and on who is travelling.
Malaria-free does not automatically mean the better safari. The Sabi Sand will out-view Madikwe on leopard almost every time. What malaria-free buys is one fewer thing to weigh up with a five-year-old in the party.

Sequencing
City first, bush last is the order that works. Cape Town and the Winelands absorb the jet lag while you are still adjusting, and the safari lands as the crescendo rather than the warm-up. Ten nights split roughly four and four, with a travel day in between, covers it without anything feeling hurried.
If the trip has to be shorter, cut the city rather than the reserve. Three nights in a private reserve is the floor below which the rhythm of dawn and dusk drives never really starts, and a two-night safari is the single most common regret we hear about.

Sabi Sand for game viewing, and for leopard in particular: the reserve permits off-road traversing and limits how many vehicles share a sighting. Kruger National Park for scale and value, with the trade-off that you stay on public roads. The wider Greater Kruger reserves sit between the two on both price and exclusivity.
